The father of Samuel’s best friend, who saw the accident, told the inquest that his son “cried in the classroom because he missed Samuel and wanted him back” and later tried to demonstrate how Samuel injured himself.
He said: “I believe there was more than one incident. I believe something has happened and then moved on and something has happened again.
“I understand from what [my son] said that he had been playing on the A-frame but also that the benches and the playground’s border played an important part as to why Sammy fell. I believe the roly poly happened on the A-frame.
“From what he said in his statement Sammy went from one bench to another bench, fell off the bench and then hit his head on the plant bit.”
Head teacher Rachael Nye said the adventure playground was installed in December 2010 and all the children knew how to use the equipment.
