John, our therapist, explained the advantages of smoking (none), and the disadvantages (lots). He invited us all to light up and then, quite out of the blue, he said: "That is the last cigarette you will ever smoke." A palpable panic hit the room. There were lawyers, shopkeepers and taxi drivers present, and the one thing we had in common was that we all felt pretty ashamed of ourselves. (It was indeed a shaming experience. I sat next to a 28-year-old man who told me that he had to give up because he had recently suffered his second heart attack, which his doctor attributed entirely to his smoking.)
